Official Rules And Conditions:
1) HOW TO ENTER
The Widescreen Review 2008 Holiday Contest (Sweepstakes) begins on August 15,2008 and ends on December 19, 2008 (the "Promotion Period.") The winner will be selected at random on December 22, 2008. To enter online, go to www.widescreenreview.com/sweepstakes and submit the online entry form to receive one (1) entry into the Sweepstakes. To enter by mail, print out the questionnaire and mail to: Widescreen Review Holiday Contest, 27645 Commerce Center Drive, Temecula, CA 92590. Online and mail-in entries must be received by December 19, 2008. Limit one entry per person, per household, and per e-mail address, regardless of method of entry. Proof of e-mailing does not constitute proof of delivery. Sweepstakes is only open to U.S. residents 18 years of age or older.
2) SELECTION OF WINNER
On December 22, 2008, one (1) winner will be selected in a random drawing that will be conducted from among all eligible entries received during the Promotion Period. The drawing will be conducted under the supervision of Widescreen Review (“Administrator”), whose decisions on all matters relating to this Sweepstakes shall be final. The odds of winning a prize will depend on the number of eligible entries received. Winner will be notified by mail, e-mail, and/or telephone.
3) PRIZES
One (1) Grand Prize winner will receive a Panasonic VIERA 46-inch 1080p TH-46PZ80 Plasma and a SC-BT100 Blu-ray Disc™ home theatre system. Please allow 6-8 weeks for delivery of prize. No substitution, cash alternative or transfer of prize is permitted, except by Sponsor, who reserves the right, at its discretion, to substitute a prize of equal or greater value in the event of unavailability.
4) ELIGIBILITY AND CONDITIONS
Sweepstakes open only to legal residents of the 50 United States (and the District of Columbia), 18 years of age or older. Void where prohibited. Employees and their immediate families of Sponsor, Administrator, and their respective affiliates, and their advertising and promotion agencies are not eligible. Sweepstakes is subject to all federal, state, and local laws. By acceptance of their prize, winner agrees to release Sponsor, Administrator, their respective families, subsidiaries, and other affiliates, and their respective advertising, promotion and production agencies from any and all liability, claims, or actions of any kind whatsoever for injuries, damages, or losses, which may be sustained in connection with the receipt, ownership, or use of the prize. By accepting prize, winner grants to Sponsor the right to use his/her name, city, and state of residence and/or photograph for promotional purposes online and off-line without additional compensation, unless prohibited by law. Sponsor reserves the right to verify eligibility qualifications of winners. Potential winner may be required to sign an Affidavit of Eligibility/Liability Release and where legal, Publicity Release within 14 days of issuance of notification. Failure to return these documents within that time period will result in disqualification of the entry and selection of an alternate winner.
5) INTERNET
An e-mail address may only be used by one (1) entrant. Subsequent attempts made by the same individual to enter by using multiple e-mail addresses or otherwise will be disqualified. Entry will be declared made by the authorized account holder of the e-mail address submitted at time of entry. "Authorized account holder" is defined as the person assigned to an e-mail address by an Internet access provider, on-line service provider, or other organization responsible for assigning e-mail addresses. By entering, entrants acknowledge compliance with these official rules, including all eligibility requirements. Neither Sponsor, Administrator, any telephone network, nor service providers, are responsible for incorrect or inaccurate transcription of entry information, or for any human error, technical malfunctions, lost/delayed data transmission, omission, interruption, deletion, defect, line failures of any telephone network, computer equipment, software, inability to access any Web site or online service, or any other error or malfunction, or late, lost, illegible or misdirected entries. Entry materials that have been tampered with or altered are void. Sponsor and Administrator reserve the right, in their sole discretion, to cancel or suspend part or all of this Sweepstakes should non-authorized human intervention, or any other cause beyond the control of Sponsor and/or Administrator, affect, corrupt, or impair the administration, security, fairness, or proper play of the Sweepstakes. In such event, Sponsor may award prizes in a random drawing from all eligible entries received up to the date of cancellation or suspension. If the Sweepstakes is suspended or terminated prior to its expiration date, notice will be posted at www.widescreenreview.com . Any attempt to deliberately damage the content or operation of this Sweepstakes is unlawful and subject to legal action by the Sponsor or its agents.
6) NAME OF WINNER
The name of the winner will be posted on or after December 22, 2008 at www.widescreenreview.com . Alternatively, you may obtain the name of the winner by mail, available after December 22, 2008, by mailing a self-addressed, stamped envelope to: Widescreen Review Holiday Contest Winner, c/o Widescreen Review, 27645 Commerce Center Drive, Temecula, CA, 92590.
7) INSURANCE REQUIREMENTS:
Commercial General Liability Insurance, including coverage for Bodily Injury, Property Damage, Personal Injury, Advertising Injury and Contractual Liability, in an amount not less than $1,000,000 per occurrence. This Policy needs to contain an additional insured endorsement and a waiver of subrogation endorsement, naming “Panasonic Corporation of North America, it’s parent, subsidiary and affiliated companies as additional insured as interests may appear for the purposes of this contract”.

Workers' Compensation Insurance in Statutory amounts, and Employer’s Liability Insurance in an amount not less than $1,000,000 per occurrence. Policy will include a waiver of subrogation endorsement, naming “Panasonic Corporation of North America, it’s parent, subsidiary and affiliated companies”.

Umbrella Excess Liability Insurance in an amount not less than $2,000,000 per occurrence, Combined Single Limit. The Umbrella requirement can be waived if the Commercial General Liability Policy provides $3 million in limits.

Advertisers Professional Liability Insurance in an amount not less them $5,000,000 providing coverage for errors, omissions, including but not limited to, oversight, failure to communicate or inadequate communication, faulty or misleading advertising, web site design and content, wrongful acts, etc related to the sponsor, creation, housing and hosting of the contest and promotion of “Make your family time in VIERA Time”. It is agreed that coverage will be maintained continuously throughout the period of this agreement and for two consecutive years, after the completion of this agreement.

Cyber Liability Coverage including but not limited to third party coverage for privacy issues, the infringement of intellectual property, virus transmission, etc.

Prize Indemnification Bonds in the amount as required in the states of FL and NY.

Widescreen Review hereby agrees to deliver to Panasonic within ten (10) days of the date of this Agreement, Certificates of Insurance evidencing the above coverage with limits not less than those specified above. Certificates of Insurance shall expressly provide that not less than thirty (30) days prior written notice to be given Panasonic in the event of a material alteration to, or cancellation of, the coverage evidenced by such Certificates, and with no disclaimer. The limits of insurance required shall not limit Widescreen Review liability under the Indemnity provision contained herein.

If sub-contractor,vendors or retailers are utilized, Widescreen Review agrees to require the sub-contractor,vendors or retailers to maintain and provide Panasonic with evidence of the insurance coverage required above.
